Program Extension Form

Please find below the necessary documents for processing your program extension. Documents that you need to download will have a "download here" hyperlink next to them. To complete each form, simply download it, print it, fill it out, scan it, and upload it. Alternatively, you can complete the forms using PDF software to fill in the necessary blanks. Please make sure that all forms are filled out completely. All financial documents that are not originally issued in English should be accompanied by a professional English translation.

Domestic Students (Certificate Program, Scholarship)

Please submit the following:

  • A copy of your scholarship award or letter.
  • A Scholarship Agreement form (download HERE).
  • A short letter to Mr. Perry outlining your plans for your second year of study and how the scholarship will help you attain your goals.
  • A minimum of half of your tuition balance for your second year.

Domestic Students (IND Programs, Scholarship)

Please submit the following:

  • A copy of your scholarship award or letter.
  • A Scholarship Agreement form (download HERE).
  • An updated Independent Study Program Handbook Agreement (download HERE).
  • A minimum of half of your tuition balance for your upcoming program.

Domestic Students (IND Programs, Non-Scholarship)

Please submit the following:

  • An updated Independent Study Program Handbook Agreement (download HERE).
  • A minimum of half of your tuition balance for your upcoming program.

International Students (Certificate Program, Scholarship)

Please submit the following:

Domestic Students:

  • 1. Certificate Program | Pay a minimum of half your tuition balance.
  • 2. IND Programs | Pay the $50 registration fee and a minimum of half your tuition balance.

International Students:

  • 1. Certificate Program | Pay a minimum of half your tuition balance.
  • 2. IND Programs | Pay the $200 extension fee and a minimum of half your tuition balance.

Please make sure that you select the correct fee from the dropdown menu. Full tuition will be due by the program start date in all cases.
In order to extend your program you must have no balance on your Peridance account. *
Are you applying for a payment plan? A 5% payment plan fee will be charged on the total balance paid after the program start date. *
